Cancellation Policy

• Cancel by September 9th for a full refund, less a $50 processing cost
• Cancel by October 15th for a 50% refund, less a $50 processing cost
• Cancel after October 15th, registration fees are fully non-refundable
• Donations, event tickets and workshops are non-refundable

Terms - All attendees must acknowledge and agree to these terms to attend:
To attend FMF (Family Medicine Forum) and/or any related meetings or events you acknowledge that an inherent risk of exposure to COVID-19 exists in any public place where people are present. By participating you voluntarily assume all risks related to exposure to COVID-19 and agree not to hold the College of Family Physicians of Canada or any of their affiliates, directors, officers, employees, agents, contractors, exhibitors, sponsors or volunteers liable for any illness, injury, disability or Public Health restrictions including, but not limited to mandatory quarantine requirements. Moreover, you also agree to follow all provincial, local and property- specific protocols such as, but not limited to, capacity limits, screening, masking, physical distancing and collection of contact information where required.

In Person Guidelines:

• You will not attend any FMF in person session, meetings or events if you are unwell
• FMF will be subject to the Province of Quebec and/or the City of Montreal’s public health guidelines and restrictions at the time of FMF
• The CFPC is not responsible for any travel, hotel or other incidental costs incurred. Please plan accordingly.

Automatic CPD Credit Entry
By checking “I agree,” I give permission to the College of Family Physicians of Canada (CFPC) to upload my continuing professional development (CPD) credits based on confirmed (scanned) and automatically tracked attendance either live, virtual or on demand in Family Medicine Forum 2023 in-person and virtual platform.

Transportation
By checking “I agree”, I recognize there are inherent risks, hazards, and dangers involved in travel and transportation in getting to and from FMF and between any events and venues offered as part of FMF. I hereby acknowledge, assume and accept any and all risks and legal liabilities related to travel and transportation including, but not limited to, possible risks of injury, loss of property, damages or death. This applies to all transportation, whether secured independently or provided or sponsored by the CFPC.

Photography
Photographs, videos and audio recordings of the FMF may be produced and may include your recognizable image. By participating in FMF, you consent to being photographed, video-taped and audio-taped and you authorize the CFPC and its partners to use any of these recordings for its activities and events, as well as for its print and electronic promotional material, including on the internet, via the CFPC or FMF websites or on other social media websites. Furthermore, you:

• agree to grant the CFPC the right and permission to use any such photographs/video or audio clips in which you may be included, in whole or in part, for such purposes;
• waive any right that you may have to inspect and/or approve any such photographs/video clips;
• transfer to the CFPC any right you may have to such photographs/video clips and waive moral rights, if any; and,
• release and discharge the CFPC from any liability that may arise from the use of such photographs/video clips by the CFPC.

All photographic materials become the property of the CFPC and may be displayed, distributed, or used by the CFPC for any purpose. Names and/or brief bios would be included with permission. Personal information obtained at this event is collected in accordance with the CFPC’s Privacy Policy.

Recordings
You may not, without the prior written permission of CFPC, copy, record, reproduce, photograph, or display any part of the In-Person or online event.

Exhibitor Permissions
By checking “I agree,” I give permission to CFPC to share my contact information with exhibitors that scan your badge during the conference. Specifically, if you allow an exhibitor or sponsor to scan your badge at their booth, you understand and agree that your contact information including name, email, company name, phone numbers, and address (as provided in registration) will be shared with that specific exhibitor or sponsor so that they may communicate with you regarding their products or services.

Additional Disclosures
In certain circumstances, we may need to share the personal information provided in registration with third parties who support FMF in fulfilling directly related event services (e.g. with hotel partners, registration providers, etc.). Disclosure of your personal information in these circumstances will be made to perform specific tasks on our behalf and is restricted using contractual arrangements that prevent third parties from disclosing or using your information for any purpose other than those with which we specify. If applicable, your address (as provided in registration) may also be shared with supplier and distribution partners, for fulfillment of any requested product deliveries.
